6. Moving from Pediatric to Adult Health Care

Introduction:

Moving into the adult health care system requires finding health care providers who are skilled and sensitive to the needs of persons with intellectual and developmental disabilities. If your family member has a rare disability, it is often difficult to find a health provider who is familiar with it. Once adult health care providers are found, there must be communication between the pediatric office and the adult health care providers. Families can have a role in making sure that happens.
