The Individualized Educational Plan (IEP) is a written plan for school with goals that you have as a student who has special education services. Your IEP can help you get ready for when you finish high school. You can include your health goals and work on these activities with your teachers, family, and others.
You may also have a community plan, sometimes called an Individualized Service Plan (ISP). Your ISP helps you get ready for living, working, and being active as an adult in the community. Since you need good health to reach these goals, it is important to include health goals along with self-determination goals in your community ISP.
